The registrar is responsible for all Birth, Death and Marriage Records within the Township, as Goverened by the New Jersey State Registrar. Through this office customers may apply for marriage/ civil union  licenses or obtain certified copies of vital records.

PLEASE NOTE: If you intend to use this form at the LOCAL East Greenwich municipal building, do NOT make out the check payable to the state; instead it needs to be made payable to East Greenwich Township.

CERTIFIED COPY OF VITAL STATISTICS
At this time, due to the Governor’s Executive Order #18, anyone wishing to obtain a certified copy of a vital statistic must be the person to whom the information pertains or a direct relation such as parent, child, sibling, grandparent or other legal guardian. They must also show proof of identification and must fill out and sign a record request form at the clerk’s office.

May be obtained from funeral director, in the municipality where the death occurred or through State Registrar

APPLYING FOR A MARRIAGE LICENSE IN NEW JERSEY 
WHERE TO GO
Apply with the Registrar of the town where the either party currently resides. Please call that municipality for times and locations.
If neither party resides in New Jersey but wish to have the ceremony here, apply in the municipality where the ceremony will take place. License will only be valid in that town.

WHEN TO GO
Apply for the license at least 72 hours before the ceremony. The license is held for 72 hours after application and is valid 30 days from the time it is picked up.

East Greenwich Twp. Registrar hours are 8:30 to 5:00 Monday, Tuesday, Wednesday and Friday with varying lunch hours. Please call in advance for an appointment due to the lengthy application process and to avoid unnecessary delays.

WHAT YOU WILL NEED
Current identification with proof of residence
Social Security Numbers of both parties
Know the city of birth of the both parties and their respective parents.
Know the names and maiden names of both sets of parents.
If either party has been married or in a civil union before, a copy of divorce decree, death certificate or annulment of the most recent marriage or union must be presented.
Name and address of person performing ceremony.
Name of municipality where ceremony will take place and date of ceremony.
Address where party can be reached after ceremony.

WHO MUST APPEAR
Both partners of the opposite sex over age 18.
One witness over 18 years of age that knows both bride and groom.

HOW MUCH DOES IT COST?
$28.00 - Payable by cash or check at the time of application.
$7.00 to receive a certified copy, Payable by cash or check at the time of application.

AFTER THE CEREMONY
After the Officiant has completed the certificate you will receive the pink copy and the person performing the ceremony will retain the blue copy for his/her records.

Please check it for accuracy again at this time, before it is registered with the State of New Jersey. The pink copy is not a certified copy of your marriage.

You must obtain the certified copy from the Registrar in the municipality where the marriage took place. Please wait at least five days before contacting that registrar. You will need a certified copy of your civil union for legal documentation for items such as insurance, banking and your driver's license. Be aware that although you may take either partners name at the time of the marriage in the state of New Jersey, that name may not be recognized by other entities such as the Federal Government. ( Social Security, Passports, etc.) You may wish to contact your attorney for further advice or seek a legal name change.

REGISTERING A DOMESTIC PARTNERSHIP IN NEW JERSEY
HOW TO REGISTER IN EAST GREENWICH TOWNSHIP
Both partners must appear together and fill out an affidavit of domestic partnership, which must be signed in the presence of a notary public. You may take this with you and have it notarized or the registrar can notarize it. Return it to the registrar with all of the proof required and the registrar will turn it in to the state..

WHO CAN REGISTER
Any couple of the same or opposite sex age and aged 62 years or older, that meet the following criteria:
1. Share a common residence
2. Live in the State of NJ or are a member in a State of New Jersey administered retirement system.
3. Are jointly responsible for each other's common welfare as evidenced by one of theitems listed in the proof requirements section.
4. Agree to be jointly responsible for each other's basic living expenses during the partnership.
5. Are unrelated to each other by blood or marriage up to and including the fourth degree of consanguinity.
6. Neither partner is currently in a marriage recognized by New Jersey law.
7. Neither partner is currently in an existing domestic partnership.
8. It has been more than 180 days since the termination of any previous domestic partnership that either partner was in. (except in case of death of a partner)


PROOF REQUIRED
1. Valid identification that establishes applicant name, age, and date of birth. Example: driver's license or birth certificate or military I.D. or passport.
2. Provide proof of joint financial responsibility as evidenced by one or more of the following: Joint deed, mortgage agreement or lease; Joint bank account; Designation of one of the persons as primary beneficiary in the other person's will; Designation of one of the persons as primary beneficiary in the other person's life insurance policy or retirement plan; Joint ownership of a motor vehicle.
3. Provide proof of common residence in the state of New Jersey. Example: same address on driver's licences, household bills with both names, lease, mortgage, etc.
4. Non-New Jersey residents must provide proof of membership in State of New Jersey administered retirement plan as evidenced by one ore more of the following: Personal Benefits Statement from the previous year; 1099R from the previous calendar year; Certificate of Pension Membership.

WHERE TO GO
A domestic partnership can be registered with any Registrar of Vital Statistics in the state of New Jersey if the couple resides in New Jersey or if one or both partners can show proof of membership in a State of New Jersey administered retirement system. Please check with the municipality that you will be going to, for registrar hours and requirements.

WHEN TO GO
East Greenwich Township Registrar hours are 8:30a.m. to 5:00 p.m. Monday, Tuesday, Wednesday and Friday with varying lunch hours. Please call in advance for an appointment due to the lengthy application process and to avoid unncessary delays. There is NO waiting period--if all requirements are met the partnership is registered immediately and you will have an informational copy to take with you. One certified copy may be produced immediately at an additional charge. Additional certified copy requests may be completed as time permits.

AFTER REGISTRATION
Each partner will be given a copy of the registration form.
Each partner will be given a rights and obligations pamphlet.
It is recommended that you request certified copies from the registrar for legal purposes at the additional cost listed above.
This is not a legal name changing process.

DIVORCE RECORDS
Divorce records are NOT maintained at the East Greenwich Township offices.

Divorce records in New Jersey for 1900-1989 are kept by the Records Information Center of the state Superior Court. Obtain a Divorce Decree or Document

For records after 1989 contact the NJ county court that issued the divorce decree.

What documents do you need for a Driver's License?
The New Jersey Motor Vehicle Commission (MVC) requires all applicants for New Jersey driver licenses, non-driver identification cards and boat licenses to present proofs of Age, Identity and Address. Applicants must also prove that their presence in this country is authorized under federal law. Without the proper documents, you may be inconvenienced and your application may be denied. **If your surname is different from the one on your primary proof of age and identity, MVC cannot complete the transaction until you document the name change with certified adoption, marriage, or divorce papers, or a name change court order. Name changes through divorce must be part of the divorce decree and/or contain an addendum signed by the hearing judge.
